housing, nutrition, mental illness, community environment, lifestyle and
employment status can affect a person’s health as well as their ability to access
the care needed to improve it.

Medicaid programs serve an ever-widening range of people and an equally wide
range of health conditions. This is a challenge in the face of increasing budget
pressure. Programs need a different approach to work through the transient
population, high membership churn, member difficulties in accessing care and
the complexity of navigating programs.
Wyoming Rewards Diabetes Education
Diabetes is one of the biggest chronic health issues in the United States today,
affecting more than 29 million people – over 9 percent of the population.

People with diabetes have higher risks of heart disease and stroke and increased
problems with activities of daily living. Diabetes is also a financial burden for
healthcare programs, with medical costs totaling $176 billion. It’s an even
bigger problem for state Medicaid programs, whose clients are twice as likely
to have diabetes.
